Rachel Colant, RN, Facility Manager

Rachel joined us during the initial startup of the clinic as a nurse manager. She has over 16 years of experience in the medical field, and extensive experience in Peritoneal Dialysis and Hemodialysis since 2011. With Rachel’s keen insight into patient needs and strong patient training experience, she will provide the one-on-one care each patient needs and deserves.

Kristi Groves, RN, Nurse Manager

Kristi joined our team in January of 2022 with 6 years of peritoneal dialysis experience. Since 2010, she has grown her nursing experience in both the hospital and clinic settings. Kristi uses her compassion and considerable knowledge to provide personalized patient training that inspires confidence in our patients.

Melissa Schwinn, Social Worker

Melissa has been a dialysis social worker since 2015. Her favorite part of her job is building relationships with her patients and their family members as a way to improve and/or maintain their quality of life while on dialysis.

Melissa helps patients gain more independence or maintain their current level of independence by encouraging them emotionally, professionally, socially, spiritually and vocationally. She also helps patients and their families navigate the often changing, confusing and intimidating insurance world.

When patients start dialysis it is life changing; Melissa is there for their transition to this new way of living, encouraging hope and a healthy lifestyle.

Raquel Rolih, Dietitian

Raquel started with Channahon Home Dialysis in September of 2020, bringing several years of dialysis experience. As a registered dietitian, Raquel has a passion for helping others live healthful lives and treating chronic diseases by optimizing nutrition intake.

Shannon Michael, Phlebotomist/Receptionist

Shannon has worked with our partner Northeast Nephrology Consultants for the last three years at their front desk. She recently transitioned to Channahon Home Dialysis as the full-time phlebotomist and receptionist position. Shannon’s positive attitude and smiling face will make anyone feel welcome.
